VO₂ Max and Lifespan: Why It Predicts How Long You Live

VO₂ max is one of the strongest predictors of mortality we have. What the research shows, what your number should be, and how to raise it at any age.

Of all the numbers you could track for a longer life, VO₂ max may be the most important. It measures the maximum amount of oxygen your body can use during hard effort, and it turns out to be one of the strongest single predictors of how long, and how well, you will live.

What the research actually shows

The evidence here is striking. In large studies, people with the lowest cardiorespiratory fitness had several times the risk of death compared with the fittest, an association as strong as, or stronger than, traditional risk factors like smoking, diabetes and high blood pressure. One widely cited analysis found that individuals with the lowest VO₂ max had roughly a fourfold higher risk of dying than those with the highest.

Just as importantly, the relationship is dose-dependent and improvable: moving up even one fitness category is linked to a substantially lower risk of death. Unlike your genes, this is a number you can change.

Why VO₂ max matters so much

VO₂ max reflects the health of your heart, lungs, blood vessels and muscles working together. A high number means your whole system delivers and uses oxygen efficiently, which underpins everything from climbing stairs to recovering from illness. As you age, VO₂ max naturally declines, and once it falls too low, everyday activities become exhausting and independence slips away. Protecting it is protecting your future capacity. This is why it is one of the four pillars of longevity training.

How to raise your VO₂ max

The good news is that VO₂ max responds to training at any age. The proven recipe combines two ingredients:

  • A big aerobic base from Zone 2 cardio, easy, conversational effort, several times a week.
  • Short, hard intervals to push the ceiling up, most famously the Norwegian 4x4: four minutes near-maximal, four minutes easy, repeated four times.
